1. When thoughts keep you up

The longer insomnia goes on, the more thoughts about sleep pile up. “What if I can’t sleep again tonight?” “I’m going to blow it at work tomorrow.” These thoughts are natural, but they set up a loop: worry raises arousal, and arousal pushes sleep further away.

This part doesn’t ask you to force your thoughts into positive ones. Instead, we look together at how accurate a thought is, and whether following it actually helps you sleep.

2. Thoughts that show up often

The thoughtAnother way to see it
“If I don’t get 8 hours, tomorrow is ruined.”How much sleep people need varies. A meta-analysis found that people with insomnia performed less well than good sleepers on tasks involving memory, working memory, and problem solving, with small to moderate differences (Fortier-Brochu et al., 2012). That finding cannot tell you how you will function after a particular night of poor sleep. Look at your own diary to see how the day actually went. That said, drowsy driving really is dangerous. If you’re sleepy, don’t drive.
“I slept badly last night, so I should go to bed early tonight.”Wanting to make up sleep makes sense. Going to bed early, though, adds time lying awake and can make the next night harder. Keeping your wake time often helps your rhythm recover.
“Everything that’s hard during the day is because of sleep.”Sleep may be a big part of it. Stress, mood, not moving much, and boredom also make days tiring. Putting it all on sleep adds more pressure to sleep.
“Everyone else manages on this little sleep.”How other people sleep, and what it costs them to keep going, is mostly invisible to us.
“I’ll sleep when I’m dead.”It’s a common saying at school and work. Sleep is what supports your focus and mood during the day.
“Taking a sleeping pill means I’ve failed.”Medication is a tool, and using a tool isn’t a test you pass or fail. You can take medication and do CBT-I at the same time. If you want to reduce or stop, make a plan with your prescriber rather than cutting back on your own. Stopping benzodiazepines suddenly, in particular, can be dangerous.
“This insomnia will never go away.”CBT-I often helps even long-standing insomnia. Your diary (Part 02) will likely show small changes first.

3. Questions to ask yourself

When a thought about sleep shows up, pick just one of these.

  • What’s the evidence for this thought, and against it?
  • On nights I slept badly, how did the next day actually go? What does my diary say?
  • If a close friend said this to me, what would I tell them?
  • If I follow this thought, will it help me sleep tonight?
  • Even if this thought is true, what’s one thing I can do right now?

The last two come from Acceptance and Commitment Therapy (ACT).

4. Your coping card

Write down the thoughts that tend to visit at night, and what you want to say to yourself when they do. Keep it on your phone’s lock screen or on a desk away from your bed. You can also make it together in session.

The thought that shows upWhat I’ll tell myself
e.g., “If I can’t sleep again tonight, that’s it.”e.g., “Tomorrow might be a hard day, but it won’t be the end. I’ll just keep my wake time.”

Write it in whichever language reaches you best, English or Korean.

Doing this together in session

Working with thoughts holds up better when it goes hand in hand with changes in behavior. At PADO, we do this work in session alongside matching your time in bed using your diary (Part 04).
