(Carney et al., 2012).

1. Why keep a diary
Bad nights stick in memory. So even when it feels like “I never sleep anymore,” writing it down often shows some okay nights mixed in. Sometimes it also shows you’ve been spending more time in bed than you realized.
In CBT-I, the diary is the map. Part 04 uses it to set your time in bed and to adjust it each week. That’s why the first one to two weeks of entries matter most.
The items in this diary are based on the Consensus Sleep Diary, a standard format agreed on by sleep researchers (Carney et al., 2012).
2. How to fill it in
- Fill it in once, in the morning. There’s no need to check the clock or write anything down during the night.
- Estimates are fine. If it felt like about 30 minutes, write 30. Consistency matters more than precision.
- Record getting into bed and turning off the lights separately. Time spent on your phone in bed still counts as time in bed.
- Dozing counts as a nap. Include falling asleep on the couch in front of the TV.
3. What is sleep efficiency?
It’s the share of your time in bed that you actually spent asleep.
Sleep efficiency = time actually asleep ÷ time in bed × 100
For example, if you were in bed for 8 hours and slept for 6, your sleep efficiency is 75%. With long-standing insomnia, this number often drops, because more of the time in bed is spent awake. In CBT-I, around 85% is commonly used as a reference point.
Still, this one number isn’t a report card for your sleep. The same number can mean different things for different people, so you and your therapist will look at the whole diary to decide next steps.
4. For the hard-to-record days
A diary can start to feel like homework, especially if you have ADHD or mornings are hard for you.
- Missing a day is okay. Blank days are simply left out of the average.
- Five items are essential. When you got into bed, how long it took to fall asleep, time awake during the night, your final wake-up time, and when you got out of bed. With just these, sleep efficiency can be calculated.
- Attach it to something you already do. Right after your morning coffee or brushing your teeth works well.
- A phone note or scrap of paper is fine. You can transfer it into the calculator before session.
5. Sleep diary calculator
Sleep Diary Calculator
Fill this in each morning, thinking back over the night. Rough estimates are fine. You don’t need exact times.
This calculator doesn’t store or send anything you enter. Everything disappears when you close the page, so use Print / Save as PDF below if you want to keep a copy. PADO can’t control shared devices, screen recordings, or saved PDFs, so please use a personal device.
| Date | ① Time I got into bed | ② Time I turned off the lights to sleep (leave blank if same) | ③ Minutes to fall asleep | ④ Times I woke up | ⑤ Total minutes awake during the night | ⑥ Final wake-up time | ⑦ Time I got out of bed | ⑧ Naps (minutes) | ⑨ Sleep quality (1–5) | Total sleep time | Sleep efficiency | Remove |

Average of the last 7 nights
Your averages will appear here once you’ve filled in two or more nights.
How do I fill this in?
- The essentials are ①, ③, ⑤, ⑥, and ⑦. These five are needed to calculate total sleep time and sleep efficiency. If you didn’t wake during the night, enter 0 for ⑤.
- ① and ②: If you scrolled on your phone in bed before turning off the lights, ① is when you lay down and ② is when the lights went off. If you turned the lights off right away, leave ② blank.
- ③ and ⑤: No need to watch the clock at night. A morning guess (“probably about this long”) is enough.
- ⑥ and ⑦: If you woke before your alarm and stayed in bed, ⑥ is when you woke and ⑦ is when you actually got up.
- ⑧: Include dozing off without meaning to.
- Days you skip are simply left out of the average.
How are the numbers calculated?
- Time in bed = ⑦ − ①
- Total sleep time = (⑥ − ②) − ③ − ⑤ (if ② is blank, ① is used)
- Sleep efficiency = total sleep time ÷ time in bed × 100
- Averages use the 7 most recent nights you entered (the lowest rows), as average total sleep time ÷ average time in bed.
Sleep efficiency is the share of your time in bed that you actually spent asleep. No single number grades your sleep. You and your therapist will decide next steps by looking at the whole diary together.
In session, we’ll look at it together over screen share.
Memory keeps the hardest nights.
A diary keeps all of them.
