Eye Movement Desensitization and Reprocessing, 줄여서 EMDR은 트라우마 치료를 위해 개발된 심리치료 기법입니다. Eye Movement Desensitization and Reprocessing, or EMDR, is a psychotherapy developed for the treatment of trauma.
핵심 아이디어는 이렇습니다. 고통스러운 기억을 떠올리면서 동시에 눈을 좌우로 움직이는 등의 양측성 자극을 받으면, 뇌가 그 기억을 "재처리"해서 정서적 고통이 줄어든다는 것입니다. The core idea is this: when you hold a distressing memory in mind while receiving bilateral stimulation — moving the eyes side to side, for instance — the brain "reprocesses" that memory and its emotional charge diminishes.
어떤 어려움에 쓰이나What It's Used For
원래 PTSD 치료용으로 시작됐지만, 지금은 불안, 공포증, 애도 같은 다양한 영역에서도 쓰이고 있습니다. It began as a treatment for PTSD, and is now used across a range of areas including anxiety, phobias, and grief.
치료 과정에서 내담자는 트라우마 기억, 그에 따른 신체 감각, 부정적 믿음을 함께 다루면서 점차 그 기억의 감정적 충격을 약화시켜 나갑니다. In the work, the client holds the traumatic memory together with the body sensations that come with it and the negative beliefs it left behind, gradually weakening the emotional impact of that memory.
여덟 단계로 진행됩니다It Moves Through Eight Phases
EMDR은 즉흥적으로 진행되지 않습니다. Francine Shapiro가 정리한 여덟 단계의 구조가 있고, 이 단계들은 치료자가 따라가는 지도 역할을 합니다. 한 회기에 여덟 단계가 다 들어가는 것은 아니고, 사람에 따라 앞의 준비 단계에 시간이 꽤 걸리기도 합니다. EMDR is not improvised. It follows an eight-phase structure set out by Francine Shapiro, which acts as a map for the therapist. Not every phase appears in a single session, and some people need considerable time in the early preparation phases.
- 과거력 청취와 치료 계획 — 무엇 때문에 왔는지 이야기하고, 다룰 기억들을 고르고, 속도를 정합니다.History taking and treatment planning — what brings you in, which memories will be worked on, and at what pace.
- 준비 — 과정과 용어를 설명하고, 처리 중에 올라올 감정을 다룰 안정화 기법을 함께 준비합니다.Preparation — explaining the process and terms, and building the coping techniques you will need when distress comes up.
- 평가 — 다룰 기억을 정하고 그에 딸린 이미지, 믿음, 감정, 신체 감각을 확인합니다. SUD와 VOC라는 척도로 시작점을 잽니다.Assessment — selecting the target memory along with its image, belief, emotion, and body sensation, and taking baseline measures with the SUD and VOC scales.
- 탈감작 — 기억에 집중한 채 양측성 자극을 시작하고, 고통 수준(SUD)이 충분히 내려갈 때까지 이어갑니다.Desensitization — bilateral stimulation begins while holding the memory, continuing until the level of disturbance drops.
- 주입 — 그 기억에 긍정적 믿음을 연결하고, 그 믿음이 온전히 사실처럼 느껴질 때까지 강화합니다.Installation — linking a positive belief to the memory and strengthening it until it feels fully true.
- 신체 검색 — 기억과 긍정적 믿음을 함께 떠올린 채 머리부터 발끝까지 몸을 살피고, 남아 있는 불편감을 처리합니다.Body scan — holding the memory and the positive belief while scanning the body head to toe, reprocessing whatever disturbance remains.
- 종결 — 처리가 끝났든 아니든, 모든 회기는 현재의 안정된 상태로 돌아오는 것으로 마칩니다.Closure — every session ends by returning to a settled state in the present, whether or not the reprocessing finished.
- 재평가 — 다음 회기는 여기서 시작합니다. 지난번에 다룬 기억이 여전히 편안한지 확인하고 다음 방향을 정합니다.Reevaluation — the next session begins here, checking that the processed memory has stayed settled and deciding what comes next.
이 세 단계에서 양측성 자극이 쓰입니다. 눈의 좌우 움직임뿐 아니라 소리나 두드림(탭핑)도 같은 역할을 합니다. 기억에 접촉하면서 동시에 현재에 발을 붙이고 있게 하는 것이 핵심입니다. Bilateral stimulation is used in these three phases. It can be side-to-side eye movements, sounds, or taps. The point is dual attention: contacting the memory while staying anchored in the present.
전제: 사람에게는 스스로 회복하는 힘이 있다The Premise: People Can Heal
EMDR이 서 있는 전제는 모든 사람에게 스스로 회복하는 능력이 있다는 것입니다. 우리는 살면서 힘든 일을 겪고, 대개는 그 경험을 스스로 또는 주변 사람의 도움으로 소화해 냅니다. 여기서 소화한다는 것은 그 일이 나빴다는 것을 알면서도, 거기서 배운 것이나 그때 내가 얼마나 버텼는지 같은 다른 정보와도 연결할 수 있게 된다는 뜻입니다. EMDR rests on the premise that every person has a natural capacity to heal. We all go through hard things, and usually we process them ourselves or with the help of the people around us. Processing here means being able to know it was a bad experience while also linking it to other information: what was learned, how much strength it took to get through.
그런데 어떤 경험은 혼자 소화하기에 너무 크거나 아프거나 충격적입니다. 그러면 그 기억은 당시의 감정과 신체 감각과 믿음이 그대로 살아 있는 채로, 마치 지금 벌어지는 일처럼 멈춰 있게 됩니다. EMDR은 이 멈춘 정보처리를 다시 움직이게 하는 작업입니다. Some experiences, though, are too large or painful or shocking to process alone. The memory then stays stuck with its original emotions, body sensations, and beliefs still active, as if the event were happening now. EMDR is the work of restarting that stalled processing.
왜 효과가 있나Why It Works
왜 효과가 있는지에 대해서는 아직 완전한 합의가 없습니다. REM 수면 중 기억이 정리되는 과정과 유사한 메커니즘이라는 가설이 유력합니다. There is no full consensus yet on why it works. The leading hypothesis is that the mechanism resembles the way memory is consolidated during REM sleep.
효과 자체는 다수의 연구에서 입증되어 WHO, 미국심리학회, 미국정신의학회, 미국 보훈부 등에서 PTSD의 근거기반 치료로 권고하고 있습니다. The effect itself has been demonstrated across many studies. EMDR is recommended as an evidence-based treatment for PTSD by the World Health Organization, the American Psychological Association, the American Psychiatric Association, and the U.S. Department of Veterans Affairs.
EMDR이 왜 작동하는지는 아직 학계에서 논쟁 중입니다. 하지만 작동한다는 것은 꽤 확실합니다. Why EMDR works is still argued about in the field. That it works is fairly well established.

I recommend Dr. Roger Solomon's demonstration of EMDR. It's a session with someone carrying medical trauma after breast cancer, and you can watch the intensity of her distress visibly drop as the session goes on.

"이게 진짜 되나?" 싶었는데, 되더군요. 훈련 과정에서 내담자, 치료자, 관찰자 세 자리에 모두 앉아 보며 이 방법이 작동한다는 것을 세 방향으로 확인했습니다. I wondered whether it could really work. It does. Over the training I sat in all three seats — client, clinician, and observer — and saw it work from each one.
작동한다는 것은 꽤 확실합니다. Why EMDR works is still debated.
