과거에 불안이나 우울장애로만 진단받았는데, 저와 만나면서 ADHD나 자폐가 스크리닝되는 경우를 자꾸 보게 됩니다. I keep meeting people who were only ever diagnosed with anxiety or depression, and in whom ADHD or autism surfaces once we start working together.
그것은 이전 상담사의 문제라기보다는, 트레이닝 자체가 신경다양성을 충분히 가르치지 않았을 가능성이 높습니다. 저도 박사 트레이닝을 받으면서 신경다양성에 대해서 따로 배우지 않았습니다. 제가 다닌 박사 프로그램에 twice-exceptionality 연구로 유명한 교수와 센터가 있었다는 점을 고려하면, 좀 충격적입니다. 박사를 받아도, 현실 임상의 세계는 수업보다 더 복잡합니다. That's less a failure of the previous clinician than of the training itself, which most likely didn't teach neurodivergence well. I wasn't taught it separately in my own doctoral training either, which is a little shocking given that my program housed a professor and a center known for twice-exceptionality research. Even with a doctorate, the real clinical world is more complicated than the coursework.

왜 불안과 우울 뒤에 숨는가Why It Hides Behind Anxiety and Depression
신경다양성이 있는 사람들은 사회적으로 오해받거나 배척되는 경험을 반복하면서, 자연스럽게 불안과 우울을 경험하는 경우가 많습니다. 그래서 상담실에 처음 도착할 때 들고 오는 것은 불안이나 우울이고, 그것이 진단명이 됩니다. 그 밑에 깔린 것은 보이지 않은 채로요. Neurodivergent people often develop anxiety and depression naturally, through years of being misread and excluded. So what they carry into the room the first time is anxiety or depression, and that becomes the diagnosis. What lies underneath stays unseen.
세상은 여전히 신경다양성에 대해서 잘 모릅니다. 저를 포함해서요. The world still doesn't know much about neurodivergence. Myself included.
어릴 때는 "똑똑하지만 산만한 아이", 청소년기에는 "예민한데 성적은 나쁘지 않은", 20대에는 "번아웃이 잦은 성실한 사람". 이 궤적을 지나온 분들이 많습니다. 자세한 이야기는 AuDHD: 서로 당기는 두 힘과 왜 여자아이의 자폐는 뒤늦게 보일까에 있습니다. "Bright but distractible" as a child, "sensitive but doing fine in school" as a teenager, "diligent but burning out often" in their twenties. Many people have walked this path. 이중문화 내담자의 평가는 왜 더 까다로운가Why Assessment Is Harder for Bicultural Clients
ASD 진단에서 bicultural한 내담자는 더 까다롭게 느껴질 때가 있습니다. In autism assessment, bicultural clients can be genuinely harder to read.
예를 들어 eye contact의 경우, 한국에서는 미국에서 기대하는 방식만큼 직접적인 eye contact을 하지 않는 것이 자연스럽기도 합니다. Take eye contact. In Korea, not holding eye contact as directly as Americans expect is often simply normal.
또 기존 ASD 진단 질문은 맥락에 따라 행동을 바꾸거나 사회적 상황에서 perform하는지를 묻는 경우가 있는데, 여러 문화를 동시에 살아야 하는 사람에게는 그 자체가 code-switching일 수 있습니다. 그런데 그 code-switching이 때로는 "사회적 수행"이나 "masking"처럼 해석될 수도 있습니다. Standard autism questions also ask whether you change your behavior by context or "perform" in social situations. For someone who has to live in several cultures at once, that is code-switching. And code-switching can be read as "social performance" or masking.
한국에서 자라 미국에 온 사람이 성인 자폐 평가를 받는 것은 그래서 정말 어려운 일입니다. 사회적 규범이 완전히 다르니까요. 사회적으로 어려워하는 것이 acculturation 때문인지 자폐 때문인지, 어떻게 분리할까요. 스몰토크나 플러팅 같은 것은 애초에 문법이 다른데요. This is why an adult autism evaluation is so hard for someone who grew up in Korea and came to the U.S. The social norms are entirely different. How do you separate social difficulty caused by acculturation from social difficulty caused by autism, when small talk and flirting run on a different grammar to begin with?
그래서 bicultural한 내담자를 볼 때는, 단순히 행동만 보는 것이 아니라 그 행동이 어떤 문화적 맥락에서 배워지고 조절되어 왔는지를 함께 봐야 합니다. So with bicultural clients, you can't just look at the behavior. You have to look at the cultural context in which that behavior was learned and regulated.
자가진단은 어디까지 유효한가How Far Self-Identification Goes
진단에 대해서 저는 생각이 복잡합니다. My thinking about diagnosis is complicated.
정식 진단을 받지 않았다는 이유만으로 그 사람의 어려움까지 가짜라고 생각하지 않습니다. 진단에는 비용과 접근성의 문제가 있고, 진단 체계가 완전한 것도 아닙니다. 자신이 ADHD나 자폐일 가능성을 발견하고 탐색하는 과정은 중요한 자기 이해의 출발점이 될 수 있습니다. I don't think someone's difficulty is fake just because it hasn't been formally diagnosed. Diagnosis has problems of cost and access, and the diagnostic system isn't perfect. Discovering and exploring the possibility that you might be ADHD or autistic can be an important starting point for self-understanding.
다만 ADHD인들에게 흔히 보고되는 특징과 ADHD의 진단 기준은 같은 것이 아닙니다. 어떤 특성이 ADHD 집단에서 더 자주 나타난다고 해서, 그 특성을 가진 개인이 ADHD인 것도 아닙니다. 이런 맥락이 사라지면 장애는 누구에게나 있을 법한 몇 가지 습관이나 친근한 성격적 특징으로 축소되고, 진단의 경계가 흐려질수록 정작 장애로 인해 삶에서 겪는 고통과 기능상의 어려움도 함께 지워질 수 있습니다. But traits commonly reported among people with ADHD are not the same thing as the diagnostic criteria for ADHD. A trait being more frequent in the ADHD population doesn't make an individual with that trait ADHD. When that context disappears, a disability shrinks into a handful of habits anyone might have, or a set of endearing personality quirks. And the blurrier the boundary of diagnosis becomes, the more the real suffering and functional impairment caused by the disability get erased along with it.
또 진단이 행동을 설명해 주는 것인지, 진단을 자신의 행동에 대한 변명으로 사용하는 것인지도 구분할 필요가 있습니다. 그 차이는 그 말을 한 뒤 무엇이 이어지는가에 있습니다. 자신의 한계를 설명하면서 필요한 조정을 찾고, 그 행동이 타인에게 미친 영향도 함께 다룬다면 설명에 가깝습니다. 반대로 "나는 이런 장애가 있기 때문에 어쩔 수 없다"고 대화를 끝내고 그 결과를 타인이 감당하게 한다면 면피에 가까워집니다. 설명은 책임의 형태를 바꿀 수 있지만, 책임 자체를 없애지는 않습니다. There's also a difference between a diagnosis that explains behavior and a diagnosis used as an excuse for it. The difference lies in what follows. If you explain your limits while looking for the adjustments you need, and deal with the effect your behavior had on others, that's explanation. If you end the conversation with "I have this disability, so there's nothing I can do" and leave the consequences for others to absorb, that's closer to an excuse. Explanation can change the shape of responsibility. It doesn't remove it.
이런 구분이 사라진 콘텐츠를 접한 뒤, 실제 삶에서 큰 기능상의 어려움을 겪는 사람들이 "내가 정말 힘든 게 맞나", "나도 그냥 변명하는 건가"라고 혼란스러워하는 모습을 봅니다. 정식 진단을 강조하는 이유는 그것이 개인의 문제가 아니라 사회적 문제라고 생각하기 때문입니다. I see people with serious functional impairment in their actual lives, who after encountering content where these distinctions have vanished, start asking "am I really struggling, or am I just making excuses?" The reason I emphasize formal diagnosis is that I think this is a social problem, not an individual one.
설명되지 않는 부분이 남아 있다면If Something Still Isn't Explained
불안이나 우울로만 진단받아 왔는데 뭔가 설명되지 않는 부분이 계속 남아 있다면, 그 남은 부분을 함께 들여다볼 수 있는 임상가를 만나는 것이 출발점입니다. 그 임상가가 꼭 저일 필요는 없습니다. 다만 신경다양성을 보는 눈과, 여러 문화를 살아온 사람의 행동을 그 문화 안에서 읽는 눈, 두 가지를 모두 가진 사람을 찾으시길 권합니다. If you've only ever been diagnosed with anxiety or depression and something keeps going unexplained, the starting point is finding a clinician who can look at that remainder with you. It doesn't have to be me. But I'd encourage you to look for someone with both kinds of eyes: one that sees neurodivergence, and one that reads the behavior of a person who has lived in several cultures inside those cultures.
